CANCELLATION POLICY
▪︎Intake Forms, Client Agreement & Identification Each client is required to complete an intake form based on the type of service received, a client contract/agreement, and submit a valid state ID. Clients are required to submit the applicable intake form, contract, and identification before any service and at the time of the appointment. The information must be filled out accurately, with a sound mind and free of any mind-altering drug, paraphernalia, legal or otherwise, to the best of your knowledge. By accepting and requesting a service from Forever Aura Skincare, you agree to release liability of Forever Aura Skincare, its Owner/Agents, product companies, and associates of any damages or harm caused as a result of failure to disclose any condition that would otherwise be considered a contraindication to any services provided by Forever Aura Skincare or its Owner/Agents. Please note that both knowingly and unknowingly, this also applies if aftercare instructions are not followed, and the client directly or indirectly contributes to damages. This includes unauthorized use of the product, not following aftercare instructions, and services with other providers without clarification that could be deemed interference of care. Updating forms with any changes is 100% the client's responsibility. It is recommended that you do so quarterly. Interference with care can be considered wax services, lash services, haircare not limited to shampooing, coloring, installation, food triggers, picking, scratching, plucking, exfoliation, injections, massaging, tattooing, and semi/permanent makeup services. Providing false and/or inaccurate information will result in permanent termination from our practice. Please also refer to cancellations due to non-disclosed medical conditions. Electronic Signature Acknowledgment By checking any box, booking any appointment, or submitting any form with Forever Aura Skincare, you confirm that you have read, understood, and agreed to all policies outlined in this document. Your acceptance via checkbox constitutes your legally binding electronic signature under the Electronic Signatures in Global and National Commerce (E-SIGN) Act and the Uniform Electronic Transactions Act (UETA), and is irrevocable. You agree to be held financially and legally responsible for adhering to all terms, obligations, deadlines, and procedures outlined within these policies. ▪︎Credit Card Authorizations and Policies I hereby authorize Forever Aura Skincare to process credit card payments from the credit card saved on file when booking. I understand that although this information is secured in an online protected client file and is unlikely to be tampered with, I agree to assume the risk if the file and credit card information are compromised. I agree that if I have any concerns or questions regarding charges to my account, or if the charge fails to post to my account, I will contact Forever Aura Skincare for assistance and/or disclosure. I agree that I will not dispute any charges with my credit card company unless I have already attempted to rectify the situation directly with Forever Aura Skincare and those attempts have failed. If I attempt to dispute charges due to fees associated with policies in place, I release my right to do so and agree to pay any additional expenses and fees applied to mine or my provider's accounts in full. I authorize my bank to honor any transactions with Forever Aura Skincare, and to rule in the favor that suits Forever Aura Skincare and its owners. I acknowledge that services cancelled, missed, late, or no-showed within 72 hours of my scheduled appointment are charged 95% of the total service fee, and are non-refundable and non-transferable, resulting in forfeiture of my booking fee. I understand that additional fees such as card fees and late fees may be applied to my service. Late fees are charged at $25 per 5 minutes, capped at $25, with the appointment slot being canceled after 10 minutes. There is one exception to this policy; please refer to the booking fees under Late Fees, Booking Fees & Additional Fee Policy. I understand that clients must have a current and active credit card on file at all times, or risk any future appointments being cancelled without notice. I understand that credit card payments will not be processed without due cause and this card will only be charged for booking fees, services, card fees, missed, late, or no-show appointment fees with or without my approval. By requesting an appointment, rebooking through my service provider, submitting appointment any requests, and accepting an appointment, I authorize Forever Aura Skincare LLC to charge my credit card for all fees associated with services booked on or after this date. I understand and agree to these terms and the conditions of this payment policy. Any attempts to place temporary holds on the card on file to avoid legitimate charges will result in permanent debarment from our practice. This policy also applies to invoiced services. I understand that all product purchases, courses, and services are final sale, non-transferable, and non-refundable. Card fees are associated with all card transactions, at percentages based on credit card processing systems. New clients are subject to booking fees of 100% of the total service, while booking fees for services are charged at 50% on a normal basis. Booking fees are transferable once within 7 days of the original slot. Beyond 7 days, the booking fee is deemed non-transferable and forfeited. A new booking fee will be required when scheduling. Loss fees will not go towards the original service balance. If I no-call no-show, arrive after 10 minutes, cancel, or reschedule within 72 hours of the appointment slot, 95% of the total service/cancellation fees will be charged, resulting in forfeiture of booking fees. ▪︎Cancellations, Rescheduling, Missed, No Call and No Show Appointment Policies Recognizing that everyone's time is valuable and appointment times are limited, Forever Aura Skincare asks that I cancel 4 days or more if I am unable to show up to my appointment. A reminder of my appointment date and time is automatically sent at the time of booking. Forever Aura Skincare encourages myself to record appointments in my personal calendar reminders to avoid confusion. Appointment reminders are issued automatically both 24 and 72 hours prior to all appointments. This reminder will arrive via email or text message. If I receive a 72 hour notice, I am aware that I am already within my cancellation window. I must therefore cancel 73 hours or more before being charged 95% of total service. By not confirming my appointment, it is not equal to an appointment cancellation. I must select the option to cancel directly through appointment notifications. Should this not be avaliable for any reason, I will follow all prompts and contact my service provider. Should I fail to read, open, block, ignore, unsubscribe or update contact information on file in order to receive communications, I understand that I will still be held accountable and do so at my own risk. If I receive initial notifications and know that I am unable to show up, I must cancel or reschedule through appointment notifications received. I only need to contact my service provider when canceling or rescheduling within 24-72 hours of any appointment slot to avoid additional fees. Appointments scheduled, canceled, missed, rescheduled, not communicated prior, or no showed within 72 hours of the appointment slot will require me to pay 95% of the total service in cancellation fees. Initial Booking Fees will only go towards scheduled appointments, and I must show up, be present in the treatment room, and be serviced at the time of my appointment to apply the Booking Fees to the appointment. Booking Fees cannot be applied to scheduled appointments if canceled or rescheduled with less than 73 hours' notice to appointments. Forever Aura Skincare reserves the right to charge 95% for any appointment scheduled or rescheduled more than twice in a row, with no notice of cancellation within the allotted time frame or less than 73-hour notice at any time. If there is a history of no-call, no-show appointments, rescheduling, last-minute cancellations, avoiding appointment confirmations, and avoiding submission of Booking Fees, Forever Aura Skincare reserves the right to refuse services in the future. If a scheduled slot is rescheduled more than twice, the full appointment balance will be due at the time of rescheduling the 3rd appointment. The card on file will be charged for the complete balance for all appointments. Any attempt to place temporary holds on the card on file to avoid fees associated with late or cancellation fees will result in permanent termination from our practice. Any client who cancels or reschedules three or more times within a 12-month period may risk being indefinitely terminated from our practice. ▪︎ Manual Booking Assistance Fee Forever Aura’s online booking platform is designed for ease and flexibility. If you request assistance with booking, canceling, or rescheduling via text, DM, or email, outside of an in-person visit, a $25 Manual Booking Assistance Fee will apply. -This fee does not apply toward your service total. -It must be paid in advance before any manual changes are made to your appointment. -Clients are strongly encouraged to manage their appointments independently via the website to avoid this administrative fee. ▪︎Cancellation Due to Non-Disclosed Health Conditions & Medication Policies If I accidentally or knowingly receive any of the following procedures or services within 14 days prior to appointments with Forever Aura Skincare I will be charged 95% of the total amount of the scheduled service: botox, filler, sun burns, permanent make up, Chemotherapy, waxing, brow services, surgeries, inability to follow protocols due to risk or any non-disclosed health conditions and medications intentional or not. I will avoid receiving listed services for 2 to 4 weeks post treatment without first notifying Forever Aura Skincare. Forever asks that you alert any other Licensed professionals you are seeking service with of any recent treatments as well. Any charges will not apply to a future appointment or products. The appointment will be cancelled, and another appointment will need to be scheduled, if allowed. Unauthorized use of products, doctor's orders without clearance, and anything considered provider interference are not permitted. This policy applies to all services. ▪︎Late Fees, Booking Fees & Additional Fee Policies Please note that Booking Fees for New Clients are subject to be charged 100% of the total service fee. These fees will go towards service, provided that I show up for my appointment on time. Please be aware that this fee is subject to change at any time without notice. When rebooking manually through my service provider, follow-up booking fees are due at the time of rebooking. If I am rescheduling a new appointment that falls after one week of my original slot, or without 73 hours notice, I understand that a new Booking Fee will be charged at the time of booking and/or check out. Any fees lost will not go towards my service balance per policy. By booking an appointment, I authorize Forever Aura Skincare to charge for Booking Fees, services, additional fees, cancellation or late fees, and associated card fees from the card on file. Once I am charged, I understand that I have one transfer if I need to reschedule within seven days of my original slot, provided that I give my provider 73 hours notice or more of the original time slot. Other than that, fees are non-transferable and non-refundable according to the policies in place. If my rescheduled appointment falls eight or more days after my original appointment slot, a new Booking Fee will be due, and previous fees will not go towards the service. This is done to ensure that I do not avoid paying booking fees associated with slots and do not bypass policies in place by skipping out on booking fees or payments in full. It is imperative that I arrive at my appointment on time. I am allotted five minutes after my appointment time, should I run late. If I am 5 or more minutes late to a confirmed or unconfirmed appointment, there is an automatic $25 late fee that will be added to the service amount. This late fee is capped at $25. After 10 minutes, the appointment slot will be at risk for cancellation, and I will may be charged 95% of the service fee per policy, forfeiting booking fees. If service can be rendered and finished in its original timeframe it will be attempted. If not, I am fully aware that Forever Aura Skincare reserves the right to cancel the appointment, applying all associated fees: 95% of the total service fee and up to $25 in late fees. If I am 10 or more minutes late, appointment will be considered a no-show, with no-show and late fees applied, and the service will not be rendered. This policy to be respectful of other clients, service providers, and their schedules. We do not rush treatments as it may compromise the client's and provider's well-being, judgement, and overall health. Payments for products, fees, courses, and services are final sale, non-refundable, and non-transferable. Booking fees are also non-refundable and non-transferable, except for one exception. If I am unable to cancel my appointment online, I understand that I will be within 72 hours of my scheduled appointment. In this case, I will be charged 95% cancellation fees and all applicable fees, including but not limited to 95% of the complete service, booking fees for rebooked appointments manually through my provider or on the booking website, cancellation fees, and up to $25 in late fees. If I do not communicate same-day cancellation to my service provider, I will be presumed to be arriving late. This will be the only other time I will be charged late fees with cancellation. Appointments cancelled online are automatically communicated to the service provider through the website. Accounts associated with any client account that is past due will be unable to book future appointments until 95% of fees can be recovered. A $25 additional fee is added per month that any fees or invoices go unpaid. Additional fees will not go towards any future appointment balances. Additional fees are capped at 90 days and $75 total. If invoices or payments go unpaid for 90 or more days, accounts in the negative will be banned indefinitely. I will be subject to and/or pursued for legal action, prosecution, and agree to cover full payment of associated bank, legal fees, and filings. Associated accounts can rebook after total fees are collected, or 90 days, whichever comes first. ▪︎ After-Hours & Holiday Appointment Fee Appointments requested outside of standard business hours (before opening, after closing, or beyond normal session duration) will incur a non-refundable $200 After-Hours Fee, in addition to your regular service total. Holiday appointments, including federal holidays, Mother’s Day, Father’s Day, Easter, or any calendar-highlighted date will incur an additional $100 Holiday Fee. This brings the total to $300 for holiday after-hours services. This fee + service total must be paid in full at the time of booking. These appointments are extremely rare and reserved for urgent needs only. Requests made casually or for convenience may not be accepted. By requesting an after-hours or holiday appointment, you acknowledge that you are asking your provider to open outside of regular availability and agree to the full surcharge. This fee is non-refundable, non-transferable, and cannot be applied toward service totals, discounts, or credits. Clients are strongly encouraged to book in advance, during regular hours, and from standard availability selections to avoid any additional fees or surcharges. ▪︎Giftcards, Additional Cards on File, Checkout All clients are authorized to place a credit card in their name only on file, unless they are a minor. If I would like to pay for services for someone else, a virtual gift card purchase is required and recommended. Clients who book with gift card purchases are still subject to all policies in place, and can be charged late fees, cancellation fees, no call, no show fees, and total service differences in addition to the gifted amount. Cards on file can be updated in person at the completion of any service. Service providers do not have access to secure, specific card details, and only the last 4 digits during checkout processes. Forever Aura accepts remainder balances of services in cash, debit transactions, and contactless Google Pay or Apple Pay transactions via card reader or card on file only. To avoid additional card fees with transactions, please bring exact cash. Account credits will not be applied, change is never carried on the premises at anytime! ▪︎Covid, Illness & Inclement Weather Related If I show any signs of illness, exposure, or am due to be tested, I will cancel my scheduled appointment and wait 2 weeks min to reschedule after positive results. Once negative results are received or I am symptom free for a minimum of 72 hours, I am free to also reschedule. If I avoid being tested, I will wait 14 days to reschedule after signs of illness have subsided. If I display any signs of illness during service, the service will be stopped immediately and full payment will be due. If I become ill and am within 72 hours of any appointment, 95% of the total payment will be due. Please note that this payment will not apply towards any future appointments or balances. Please also be aware that discretion will be provided under rare circumstances. If I have to cancel or reschedule due to inclement weather conditions, I will do so within the time allotted to avoid any associated fees. I understand that forecasts and conditions are public information and usually provide up to 14 days of advance notice should I need. If I am unsure of my personal ability to travel and arrive safely, I will cancel and/or reschedule before I am within 72 hours of the scheduled service. Please note that if the City is specifically shut down due to any Government Ordinance with advance public notice, I will still be held completely responsible to Client Policies in place. ▪︎ Loyalty, Birthday & Referral Redemption + Rewards & Discount Policy Redemption offers cannot be combined with other promotions, transferred, or applied toward Microneedling, Green Peel services, or subscription-based treatments. If I cancel, reschedule, or miss my redeemed slot, I forfeit the reward completely. A new card will be issued with a new opportunity to build and redeem. Should I choose to redeem points, I will inform my provider at the time of booking or select the appropriate option manually online. All redemption requests must be scheduled in advance—rewards cannot be applied retroactively or at checkout. Birthday Month Redemptions are valid only for active clients (seen within the last 60 days), and not eligible for first-time visits. Referral redemptions are applied after the referred client receives their first paid service. Loyalty cards are for non-membership clients only. Membership clients earn milestone rewards and benefits separately. Clients may redeem one promotional offer per appointment. Rewards, discounts, redemptions, and incentives cannot be stacked, swapped, exchanged, or applied retroactively unless explicitly stated. Membership benefits stand independently and are not combinable with loyalty cards, birthday rewards, or promotional discounts. Military Discount: $10 off services under $130 $15 off services $130+ (Valid military ID required) Military discounts may not be combined with loyalty rewards, redemptions, bundles, memberships, or limited-time promotions. Forever Aura Skincare reserves the right to update this policy at any time to maintain fairness and protection for both clients and providers. ▪︎Packages, Memberships, and Installment Plans Please note that for any cancellations or rescheduling of services, including loyalty treatments, covered under Packages, Memberships, and Installment Plans, a minimum of 73 hours' notice is required. If proper notice isn't provided, the scheduled services will unfortunately be forfeited, not performed and are nonrefundable. This is because the time slot reserved for you could have been offered to another client. These services are not excused from policies in place. I acknowledge that if I begin a treatment installment plan tailored for optimal results, it is essential for me to complete my initial treatment within 30 days from the plan's commencement. An estimated end date will be provided, and any treatments not completed as per the prescribed schedule by the given end date will regrettably be forfeited, not scheduled, or performed as to not compromise the integrity of the skin and not sticking to schedule. Clients that avoid treatment plan as professionally recommended may not achieve wanted results, and take full responsibility for their own schedule. Bundle pricing may not be combined with any other discounts, loyalty, referral, or birthday rewards. Package bundles must be purchased in full at the time of booking to receive discounted pricing. Bundles are not retroactive, and unused sessions may not be applied toward future packages or redeemed later for a lower rate. All package sales are final. No refunds will be issued once the package is purchased. Cancellations or missed appointments within the bundle are subject to the same policies as individual sessions and may result in forfeiture. ▪︎ Late Change Eligibility & Fee Policy Clients on installment plans, prepaid packages, or memberships are eligible to reschedule an appointment within the 72-hour cancellation window by paying a $50 Late Change Fee. -This fee does not go toward the cost of your service. -It must be paid before rescheduling. -If the Late Change Fee is not paid at time of booking, the service will be forfeited and marked as redeemed. This policy is in place to preserve the integrity of prepaid sessions and maintain accountability for all clients. Any additional fees (Manual Booking Assistance, Late Fees, etc.) are separate and do not apply toward the Late Change Fee or service total. ▪︎ Membership & Subscription Cancellation Requirements Membership and Subscription cancellations must be submitted in writing to foreverauraskincarellc@gmail.com at least 30 days (memberships) or 7 days (subscriptions) before your next billing cycle. You may also schedule the appropriate cancellation “service” through our online booking site to generate a timestamped receipt. Both email and booking are required for full documentation. Verbal cancellations, social media messages, or unconfirmed cancellations through the booking system alone will not be honored. Both email and scheduled cancellation service are required. Membership clients are not eligible for loyalty points, as benefits are built into your monthly plan. ▪︎ Missed Appointments & Membership Fees Your monthly membership fee is required whether or not you attend your scheduled treatment. Memberships function like a gym membership, fees are paid to hold your slot and retain access to benefits. Missed, canceled, or no-show appointments do not excuse the monthly payment and do not roll over. ▪︎ Milestone Reward Expiration Rebirth members receive one 50% off reward on a corrective service valued at $400 or less at the completion of their 6-month term. This reward must be redeemed within 30 days of their 6-month milestone date. After 30 days, the offer expires and is no longer valid. This reward cannot be carried into a new contract term or used after cancellation, downgrade, or lapse in membership. ▪︎ Glow Essentials Subscription Terms Glow Essentials Subscriptions are prepaid in 3-month cycles and automatically renew unless canceled in writing. Requests made fewer than 7 days before billing will result in a new 3-month cycle. No mid-subscription changes, refunds, or rollovers are permitted. Clients are responsible for managing their subscription renewal timelines. Missed cancellations will not be reimbursed or paused. Subscription benefits end when the term concludes or payment fails. Doesn't include loyalty points or service rewards unless purchasing entire full-size regimen. ▪︎ Policy Revisions & Enforcement Forever Aura Skincare reserves the right to update, revise, or enforce these policies at any time, with or without prior notice. It is the client’s responsibility to review the most current version of the policies, which are always available on the booking platform and updated on client intake forms. Continued booking and engagement with Forever Aura Skincare constitutes agreement with the most recent policies.
